Register as a new patient:

Our practice area encompasses all addresses within an approximate 1 mile radius of the surgery. Patients who have moved into our practice area and who are not registered with a local doctor may apply to register with out practice by completing a registration form.
The practice will not be able to register patients who do not meet the above criteria

If you are a Temporary Resident please see below.

New Patients: If you would like to register with our practice, please bring your medical card to the surgery if possible. Please note, however, that our patient registration policy does not allow us to accept patients who are already registered with a local GP

If you do not have your medical card, please fill in our permanent patient registration form or new-born baby registration form.

To register, please come to the surgery and request the registration pack. All new patients aged between 40 and 70 will be asked to attend a health check appointment to complete registration.

Temporary Residents:

The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to complete our Temporary Resident Registration form which is available from reception.
